During two home visits, we review all aspects of aging safely—home safety, supports, transportation, meals, health needs, funding options, and more. You’ll receive a detailed Wellness Plan tailored to your individual situation, complete with local resources and actionable next steps.

Ongoing support for a smooth, confident transition.
